本期NEJM刊发了一例非常有趣的病例1:一例74岁男性肛管鳞状细胞癌,9年前患前列腺癌,曾经接受碘-125近距离放疗治愈。尽管肛管鳞状细胞癌在中国较少,但这个病例的诊疗过程还是能给国内同行带来很多有益的思考。

病历陈述


Theodore S. Hong医师:一名有前列腺癌病史,曾接受过短距离放射治疗的74岁男性因持续6个月的直肠出血在本院门诊接受诊视。

之前患者的健康状况一直良好,直至此次检查前大约6个月时发现排便时有少量红色鲜血。血液不与大便混合在一起。此次检查前2个月,患者的大便直径变小。

检查时,患者自诉每日排便,并且没有腹痛或体重下降。患者3年前的结肠镜检查结果正常,并且无直肠或腹部手术史。此次检查前9年,常规筛查发现患者的前列腺特异性抗原水平升高,并进行了前列腺活检。
