Case 12-2017 — A 34-Year-Old Man with Nephropathy

Daniel H.Katz医师(内科):一名合并听力障碍的34岁男性因肾功能恶化而被收入本院。

直至此次入院前约11周,患者一直身体健康。当时患者自觉发热,并伴出汗,还出现疲劳、食欲下降以及恶心等症状。他最初认为这些症状与天气炎热有关;这些症状在接下来的2个月内加重。在那段时间,患者出现咳嗽和劳累性气促症状,而且患者还注意到他的尿液呈泡沫状。据报告,他体重下降了9 kg。

在此次入院前22日,患者到另外一家医院急诊科接受评估。血肌酐水平为0.8 mg/dL(71 μmol/L;正常范围,0.6~1.5 mg/dL [53~133 μmol/L]),血尿素氮水平为7 mg/dL(2.5 mmol/L;正常范围,8~25 mg/dL [2.9~8.9 mmol/L])。患者未经医师评估便离开医院。
