胰岛素泵治疗1型糖尿病

患者男性,39岁,1型糖尿病病史27年,就诊于他的内分泌科医师,检查血糖控制情况。该患者超重(体质指数为28.4)。总体血糖控制不甚理想,最近几年糖化血红蛋白波动于7.5%~8.0%。患者在自我血糖监测中出现不可预知的血糖波动且频繁发作的低血糖事件,已经严重影响他的工作及家庭生活。与之前相比,低血糖的发生现在已很少出现警示症状,尽管患者已竭尽全力通过皮下注射胰岛素强化治疗、糖尿病诊所定期复诊及听取糖尿病护士的建议。1年前参加了一个系统的糖尿病教育课程,发现该课程非常实用,糖化血红蛋白的控制水平略有改善,但低血糖事件依然频繁发生。内分泌科医师已排除其他疾病,包括腹腔疾病和Addison征,由于血糖控制欠佳,医师考虑使用胰岛素泵治疗可能更为合适。由于他的内分泌科医生没有太多使用胰岛素泵降糖治疗的经验,于是推荐他去专业胰岛素泵治疗中心诊治
