每年,全球有超过50万人被诊断为肝细胞癌,其中美国新发病例约为20,000例1,2。肝癌发病率在男性常见肿瘤中排第五位,在女性中为第七位。大多数肝癌(85%)发生在发展中国家,包括东南亚和撒哈拉以南的非洲地区等乙型肝炎病毒(HBV)感染高发区域(图1)3。肝细胞癌在40岁前鲜有发生,之后逐渐升高,至70岁达到顶峰,其中男性发病率是女性的2~4倍。在美国,丙型肝炎病毒(HCV)感染是导致癌症相关死亡快速增加的主要原因,在过去的20年中,美国肝细胞癌的发病率增加了3倍,但是5年生存率仍低于12%2(图2),其中,45~60岁的西班牙语裔和白种人是增加比例最大的人群4
