50年前,Ashbaugh及其同事对12例感染或创伤后出现呼吸急促、难治性低氧血症以及胸片显示弥漫性阴影的患者进行了阐述1。在7例死亡患者中,发现6例患者的肺泡腔内被覆明显的透明膜,在此之前,肺泡腔内透明膜被认为是新生儿呼吸窘迫综合征的特异体征。因此,Ashbaugh提出了成人(后来改为急性)呼吸窘迫综合征(ARDS)一词。


自17年前NEJM发表最后一篇ARDS综述2以来,在受累患者以及存在ARDS患病风险患者的治疗方面取得了实质性进展,ARDS的发病率和死亡率均有所下降。但尽管如此,ARDS仍然是比较常见的导致患者死亡或失能的综合征。在近期一项涉及29,144例患者的国际性研究中3,10%的重症监护室(ICU)患者以及23%的行机械通气患者存在ARDS。患重度ARDS的患者死亡率为46%3。ARDS幸存患者出现认知减退、抑郁、创伤后应激障碍和持续性骨骼肌无力的风险较高4,5
