The Future of Care — Preserving the Patient–Physician Relationship

在21世纪,美国医师越来越多地被夹在两种相互冲突的观念之间:医生是人道主义职业,而医疗业则是充满竞争的商业。作为有幸领导梅奥医学中心近十年,近期刚刚退位的执业神经科医师和医师研究者,我有一些经验和见解,可能有助于我们的职业回归其核心价值和重中之重。

在当今复杂变化的医疗环境中,医学的最基本要素仍然是医患关系。不论文化和时代如何变化,医患关系始终居于医疗的核心地位,而且我认为,即使医学不断发展,这一关系也必须在医疗实践中保持核心地位。为了确保这一点,我们可以倡导支持这一关系的两项重要原则。首先,至关重要的一点是让医师有充足的时间诊治需要投入较多时间的患者,例如诊断结果不明确的患者、治疗计划失败的患者及走到生命尽头的患者。其次,对于到多名专科医师处就诊的患者,我们需要确定一名协调医师并支持其工作,以确保各位医师之间的充分交流,并且在有分歧的情况下阐明诊疗计划,并协助患者决定下一步行动。通过共同努力,医师领导和执业医师可以采取措施,将这两项原则付诸实践。
